5 KPIs every home-care agency owner should track

Most owners track revenue and headcount. Those matter, but they’re lagging indicators — they tell you what already happened. The five KPIs below tell you what’s about to happen, so you can fix problems before they show up on the P&L.

1. Time-to-fill (days). From requisition open to candidate signed offer. Track by credential type; CNAs and HHAs move at different speeds.
2. Cost-per-hire. Total recruiting spend divided by hires in the same period. Compare against the $400–700 industry benchmark.
3. 90-day retention. The percentage of new hires still on the roster three months in. Below 70% means your hiring bar or onboarding is broken.
4. Caregiver utilization. Billed hours divided by available hours. The single biggest lever on gross margin.
5. Lead-to-hire conversion rate. How many of the candidates you contact actually convert. Below 10% suggests your follow-up speed or pitch needs work.

Marketing your agency on a budget

Google Business Profile. Claim and fully fill out your profile. Post a photo weekly. Respond to every review within 48 hours. This is the single highest-ROI marketing investment for most local home care agencies.

Discharge-planner outreach. Build a relationship with two or three discharge planners at your local hospitals and SNFs. Bring them cookies, ask what they need, send a one-page PDF about your services. Referrals compound.

Family caregiver referral playbook. The adult children of current clients are your warmest leads. A short, well-timed email after each successful shift — “Did Mom have a good week? Know anyone else we could help?” — converts better than any paid channel.
